Great startup service and/or backup credit card processor. Add this service to a small practice to take credit cards right away on a website. Wells Fargo based. Last I looked, it was only $60/year with slightly higher percentages -- a great deal that I appreciate now that I have the expensive full service processor! Note: max $500/charge, max $3000/month for basic, last I checked. Higher percentage fees are totally made up for by the low annual fee. I used this service for a long time before upgrading to the next one, below, and this a great solution for starting starting to take cards. (Remember to offer package deals to your clients now that you can take plastic!)

Online Shopping Cart System & List Management Service
Practice Pay Solutions (intermediate to advanced)
Online commerce for intermediate to high-volume businesses: list management and shopping cart combined. This is a privately branded version of 1ShoppingCart, same functionality and interface as KickStartCart, BlissCart, and others. Interface is not intuitive; helps to have a virtual assistant to set up all your autoresponders, products, etc. Note that in addition to this shopping cart system, you also need a merchant account and a gateway/bridge service like Authnet.com. They total about $100/month for me.
